Thanks to incredible trans women like Munroe Bergdorf and Laverne Cox, more and more people are feeling empowered to change their biological form to match their gender identity. But what is it like being (and dating as) a trans man? I chatted to pansexual trans man, J, heterosexual trans male, K, and non-binary, transmasculine person, Cas, to ask them what questions they’re constantly asked by cis people. FYI, these kinds of questions can be intrusive, offensive and disrespectful – so please, just don’t’ ask them.

1. “Aren’t you just a lesbian?”

Urm, can a man be a lesbian? In short, no! J describes the difference between sexual identity and gender identity as“two distinct things”. J explains, “Gender is who you are. Sexuality is who you do.” Some trans men can even find a sexual awakening once they begin their physical transition. K describes himself as a heterosexual male.

“I would have dreams about marrying women and being their prince,” he says. “But I just attributed that to an overactive imagination. Once I found the language to describe the discomfort I was feeling, I began to slowly love myself enough to start seeing myself as a sexual being. At that point, I started realising that I was very attracted to women.”

2. “When are you going to have surgery? Do you have a dick?”

Trans men go through different stages of transition. And not all trans men want to make physical changes to their biological form, instead choosing to transition socially. For other trans men, physical changes aren’t an option. In the UK, gender confirmation surgery is covered by the NHS. Wait lists can be long though, and require a ‘social gender role transition period’ (a period of time living as the gender you want to transition into) of 1-2 years prior to surgery.

K, who’s living in the US, is currently unable to take steps to physically transition. “I plan on doing all of these things, I just have to wait until I’m financially and safely able to do so due to my personal situation between me, family, and work.”

Being unable to transition physically can lead to being misgendered, which can be very upsetting. “Trying to find someone ‘willing’ to date a trans man is challenging, especially if you're pre-T (testosterone, a male hormone taken by trans men during physical transition) pre-op, etc. A lot of times I get, ‘Oh, sorry I'm not into girls’, which is incredibly frustrating,” K continues. “Any time I face rejection from someone, I constantly worry whether the person genuinely wasn’t interested because we weren't meshing well, or if it's because I'm trans.”

Luckily for K, he found a partner who helped him through the early stages of his transition. “She bought me my first pair of boxer briefs, and encouraged me to get a binder and stop shaving my legs and armpits. Thanks to the support of her and my friends, I began to become more confident with my body, and felt like I was able to be sexual without nearly as much insecurity.”

3. “Do you like having sex all the time?”

For some trans men, especially those who haven’t yet began their physical transition, sex can be a difficult subject. As Cas explains, their biological body affected their sexuality, “I actually identified as asexual for many years. Looking back on it now, this came from a combination of gender dysphoria (a term used to describe discomfort at someone’s biological identity being different to their gender identity) and anxiety. I'm not saying this is the case for everyone who identifies as asexual, but I had a lot of internalised transphobia.”

They mention that this was because they felt “repulsed” by their biological form, but not understanding why. “Trans people are often either hypersexualised, or totally desexualised,” they explain. “And I went for the latter, embracing it as a form of self-protection. I thought that if I said that I was asexual, then I would stop people from sexualising the body that I struggled with so much.”

4. “Will taking testosterone just make you more angry?”

Many trans men who take T explain it’s like going through a ‘second puberty’. As well as physical changes like increased hair growth, periods stopping and even changes to muscle formation, there can also be some emotional changes too - just like being a teenager. This can be challenging in relationships. J says, “It’s helpful to understand that when we begin hormone treatment, it is basically second puberty, so forgive us for acting like moody teenagers at times.”

Just like a relationship between cis-gendered people, if you’re dating a trans man, it’s important to check in with each other about how you’re feeling. Taking hormone replacement therapy (HRT) is an important step on the road to a physical transition, and if you’re dating a trans person, be aware they might need supporting through these changes.

5. “Are you more ‘in touch with your feminine side’ than cis men?”

Some trans men feel that because they’ve experienced life with a female-assigned body, they understand more about what life as a woman is like. J says he likes to think he’s more empathetic, and aware of his behaviour. “We've lived lives where people saw us as women, and experienced the misogyny, cat calls, and sexual harassment that women go through.” He’s adapted his behaviour to make women feel more comfortable around him in the past, but knows that not all trans men do the same.“Some trans men can get caught up in the toxic masculinity, however, as we do feel that we have to act or behave in certain ways to be seen as a man.”

Cas agrees, saying, “There's a temptation for people to say that trans guys are more sensitive, understand misogyny better, and are more in touch with their emotions. That may be true for some, but don't take it as read; get to know a guy first!”

6. “How do you have sex?”

Ugh, this old chestnut! Sex comes in many different forms. When taking T, the clitoris can get bigger and increase in sensitivity, leading to some serious pleasure. For some trans men who take T, some of these physical changes can be difficult to get used to.

“It's a lot more sensitive than it used to be, and we can end up enjoying different things sexually, as well as experiencing dryness down there,” J says. “Since transitioning, I've had the best sex of my life, met the best partners, and I’m the most comfortable I have been, especially when trying new things and switching roles.”

Some trans men who don’t take T can find sex difficult. K tells me that it’s all about communication. “As a result of not being on T and not having the ‘proper equipment’, I don't like receiving pleasure from my partners. To compensate, I tend to be a giver... I guess it's just dependent on the person, and the roles they like to take on in their sexual relationships.”

A trans man is a man who was assigned female at birth. The label of transgender man is not always interchangeable with that of transsexual man, although the two labels are often used in this way. Transgender is an umbrella term that includes different types of gender variant people (including transsexual people). Trans men have a male gender identity, and many trans men choose to undergo surgical or hormonal transition, or both (see sex reassignment therapy), to alter their appearance in a way that aligns with their gender identity or alleviates gender dysphoria.^[1]

Although the literature indicates that most trans men identify as heterosexual (meaning they are sexually attracted to women),^[2][3] trans men, like cisgender men, can have any sexual orientation or sexual identity, and some trans men might consider conventional sexual orientation labels inadequate or inapplicable to them, in which case they may elect to use labels like queer.^[4]

Terminology

The term trans man is used as a short form for either identity (transsexual man and transgender man).^[5] This is commonly referred to as female-to-male (FTM or F2M).^[6]Transgender man is an umbrella term that may include anybody who was assigned female at birth (AFAB), but identifies as male.^[7] For instance, some androgynous, bigender, and genderqueer people might identify as transgender.^[5] Because transgender is an umbrella term, it can be imprecise and does not always describe specific identities and experiences.^[8]Transmasculine is a general, broader term for individuals who were AFAB but identify closer to the masculine (or male) side of the gender spectrum.^[9][10]

The term transsexual originated in the medical and psychological communities. However, unlike the term transgender, transsexual is not an umbrella term, and many transgender people do not identify as such.^[11]Transsexual is a term for AFAB and assigned male at birth (AMAB) people alike who feel their sex organs do not reflect their gender and have chosen to change some aspect of their body; it is an older term, with GLAAD stating that it is "still preferred by some people who have permanently changed – or seek to change – their bodies through medical interventions (including but not limited to hormones and/or surgeries)."^[11]

Originally, the term trans men referred specifically to female-to-male transsexual people who underwent hormone replacement therapy (HRT) or sex reassignment surgery (SRS), or both. The definition of transition has broadened to include theories of psychological development or complementary methods of self-acceptance.^[8][12] Many of those who identify as transgender may face gender dysphoria.

Transsexual and transgender men may seek medical interventions such as hormones and surgery to make their bodies as congruent as possible with their gender presentation. However, many transgender and transsexual men cannot afford or choose not to undergo surgery or hormone replacement therapy.

Many who have not undergone top surgery choose to bind their breasts. There are a few different methods of binding, including using sports bras and specially made binders (which can be vest-type, or wrap-around style). Tape or bandages are often depicted in popular culture.

Some trans men might also decide to pack, to create a phallic bulge in the crotch of clothing. However, this is not universal. Trans men who decide to pack may use anything from rolled up socks to specially made packers, which resemble a penis. Some packers are also created for trans men to be able to urinate through them (stand-to-pee, or STP, devices), or for sexual penetration or other sexual activity (known as "pack-and-play").

Transitioning might involve some or all of the following steps:^[13]

Being socially accepted as male (sometimes known as passing) may be challenging for trans men who have not undergone HRT and/or surgery.^[15][16] Some trans men may choose to present as female in certain social situations (e.g. at work).^[15][16] After physical transition, trans men usually live full-time as male.^[15][16] However, some transmasculine individuals might choose to use and engage their bodies to be pregnant, birth a baby, and breastfeed.^[9][17]

Prevalence, identity and relationships

In the United States, the ratio of trans men within the general population is unclear, but estimates range between 1:2,000 and 1:100,000.^[18][19][20] A U.S. Census Bureau study in 2015 suggests that there were around 58,000 name changes in census records consistent with female to male transitions although only 7,500 of these changed their sex coding as well.^[21]

In a study by Kara Devaney, entitled Transgender Research Literature Review, it is addressed that the term transgender encompasses a myriad of different and unique identities that do not follow the "normal" rules of gender. Miriam J. Abelson writes, "There is no question that trans men's experiences are men's experiences and give insight about men, masculinity, and gender inequality."^[22]

Like cisgender men, trans men can have any sexual orientation or sexual identity, including heterosexual, gay, bisexual, and queer,^[23] and some trans men consider conventional sexual orientation labels inadequate or inapplicable to them.^[24] The literature commonly indicates that sexual attraction to those of their same gender (e.g., trans men liking men and trans women liking women) is considerably less common among trans men than among trans women; the vast majority of trans men are reported as heterosexual.^[2][3] Surveys from the National Center for Transgender Equality show more variation in sexual orientation or sexual identity among trans men. In NCTE's 2015 Transgender Survey of respondents who identified as trans men, 23% identified as heterosexual or straight. The vast majority (65%) identified their sexual orientation or sexual identity as queer (24%), pansexual (17%), bisexual (12%), or gay/same-gender loving (12%).^[25]

Some trans men date heterosexual women, while other trans men date queer-identified women; the latter might be because queer-identified women are less invested in the gender and sexual anatomy of a person when it comes to selecting an intimate partner.^[3] It is also common for trans men to have histories with the lesbian community or to feel that they identify better with that community because of its wide acceptance of gender variance, with a number of trans men having identified as lesbian (often as a "butch lesbian") before realizing that they are instead transgender.^[2][3][26]

Trans men have less success integrating socially within cisgender gay men's communities, which tend to be more body-focused, especially in terms of being phallocentric.^[3][27][28] Yitzchak et al. state that, as a result, they more commonly see gay trans men partnering with each other than with cisgender gay men.^[3] There are, however, cases of women being likelier than men to thoroughly question trans men about their motivations for modifying their bodies.^[27]

Some scholars argue against assumptions that trans men are predominantly heterosexual and usually have lesbian histories. In scholars Ian Irving and Rupert Raj's book Trans Activism in Canada, researchers state, "There is still a common misperception that trans men are largely heterosexual amongst those who conflate gender identity and sexual orientation. It is frequently assumed that trans men are exclusively attracted to women and have lesbian histories prior to transition." They add, "Recent data from the Trans PULSE project (Bauer, Redman, Bradley, & Scheim, 2013) challenge this assumption, with 63 percent of female-to-male spectrum trans people in Ontario reporting non-heterosexual identities and/or past-year sex with trans or non-trans men." They also argue that, based on some research, "many non-trans gay men have welcomed trans men into gay communities and have increasingly recognized trans men as potential sexual and romantic partners."^[29]

Health

Accessing health care services for trans men can be difficult.^[30] HIV infection between trans men and others is still increasing.^[31]

Alphonso David, the Human Rights Campaign Foundation President, noted that in the United States, “at least 37 transgender and gender non-conforming people were victims of fatal violence” in 2020—far more than has been recorded in previous years. But anti-trans violence is not just physical but also psychological, a symptom of the transphobia that is prevalent in our society.

The subtlety of this negativity is manifested in a variety of ways, including during interpersonal interactions—such as our willingness to date a trans person. Who we date (or don't date) can be tainted by our susceptibility to societal attitudes. “One such attitude that may be restricting the roll call of those we consider acceptable dating partners may be cisgenderism… the ideology that views cisgender identities as natural and normal, thereby delegitimizing trans identities and expressions.”

Researchers Karen Blair and Rhea Hoskin (2019) addressed the dating preferences of nearly a thousand online participants with the question, “Who would you consider dating?” Options were cisgender man, cisgender woman, trans man, trans woman, and gender queertrans. The participants were predominantly young adults, most of whom were straight, cisgender individuals (their current gender identity matches the gender they were assigned at birth) residing in Canada and the United States.

Extremely few—less than 3 percent—of straight men and women would consider dating a trans individual, regardless of whether that person matched their straight sexual orientation (a transman born female for straight men; a transwoman born male for straight women) or their gender preference (a transwoman for straight men; a transman for straight women). Unfortunately, participants were not explicitly asked about their reasons for choosing a dating partner.

Gay men were more willing than straight men (12 percent vs. 3 percent) and lesbian women were more willing than straight women (29 percent vs. 2 percent) to date a trans person. Overall, gay men were far more likely than lesbians to exclude individuals based on their trans status. Both gays and lesbians were, however, considerably more likely to date a trans person consistent with their preferred gender presentation rather than their preferred genitalia (transmen for gays, transwomen for lesbians).

We do not know the importance of whether that dating partner had altered their genitalia through surgery to match their trans identity. That is, how important is it to a gay man that his transman date does or does not have a penis or to a lesbian woman that her transwoman date has or does not have a penis? These issues require further investigation, beginning with intensive interviews with all relevant participants.

As one might expect, bisexual, queer, and nonbinary individuals were most likely to date a trans person—slightly over half. Yet, one might wonder why this was not closer to 100 percent. Despite the commonly held assumption that bisexual, queer, and nonbinary individuals have no or few sexual or gender preferences, this belief is, I believe, mistaken. Indeed, most have a favorite sex and gender of the person they desire to have as a partner. For example, regarding bisexuals, dating for trans guys, research clearly shows that relatively few bisexuals are evenly divided in their sexual preference between males and females; rather, they have a clear preference for one or the other (Savin-Williams, 2021). So, too, although gender preferences are less frequently investigated, it appears that many bisexuals have a decided predilection for the gender presentation (masculine or feminine) of their dating partner, dating for trans guys. Bisexuals who display no sexual or gender preferences are technically pansexuals; in the current study, pansexuals might have identified as queer or nonbinary.

Perhaps the most surprising finding in the Blair dating for trans guys Hoskin study is that a large number of queer and nonbinary individuals would not date their “own kind.” I admit, however, that the number of these individuals is difficult to determine because they were combined with bisexuals in the data analyses. The authors noted that the number of trans-identified individuals was too small to find patterns.

I believe we should not be surprised that even though individuals might not self-identify as male, female, masculine, or feminine, that would not necessarily preclude them from preferring a particular sex or gender presentation in their dating partner. Trans men have less success integrating socially within cisgender gay men's communities, which tend to be more body-focused, especially in terms of being phallocentric.^[3][27][28] Yitzchak et al. state that, as a result, they more commonly see gay trans men partnering with each other than with cisgender gay men.^[3] There are, however, cases of women being likelier than men to thoroughly question trans men about their motivations for modifying their bodies.^[27]

Some scholars argue against assumptions that trans men are predominantly heterosexual and usually have lesbian histories. In scholars Ian Irving and Rupert Raj's book Trans Activism in Canada, researchers state, "There is still a common misperception that trans men are largely heterosexual amongst those who conflate gender identity and sexual orientation, dating for trans guys.
